In conjunction with my participation in the Brisbane Open 2023 GLTA Masters Series Event (BO 2023)
and the related social activities and events (Activities), I agree to waive any and all rights and claims
for bodily or emotional injury, injury to my reputation, or any other damages or harms that I might
suffer, or any other claims of any nature I may have against the Bo 2023 organizing committee, the
Gay and Lesbian Tennis Alliance (GLTA), and their officers, Event Host, Tennis Australia, or any
organization, facility or individual associated with this tournament to the maximum extent as
permitted under Australian Law. I also waive any and all rights to claim from the BO 2023 organising
committee, GLTA, their officers, or any organisation or individual, damages or remuneration which
may arise as a result of references to or photographs of me that may appear in a publication or other
media coverage.

I acknowledge that uncivil or inappropriate behaviour may, at the discretion of the tournament
director, result in my expulsion from the BO 2023.

In consideration of the acceptance of your admittance by Queensland Tennis Centre (QTC) as a Guest,
you hereby agree on your own behalf, (and on behalf of your personal representatives, heirs,
executors, administrators, successors and assigns) that you hereby forever release and discharge QTC
and its officers, directors, agents, employees, independent contractors, affiliates, successors and
assigns from any and all claims, damages, injuries, losses or causes of action (whether known or
unknown to you),
(a) which may arise from your participation in Activities;
(b) which may arise from your use of any of the facilities for any of the Activities;
(c) which may arise from the use of any service offered by any independent contractor at any venue
where an Activity is scheduled; and
(d) which may otherwise arise from your attendance or presence including, without limitation, any
claims, damages or causes of action arising from the negligence of QTC’s staff, employees, officers,
directors, agents, representatives or independent contractors performing services at any Club;

For the same consideration, you also agree that you will indemnify and hold QTC & TBT Inc and the
GLTA harmless from any and all claims, damages or causes of action which may be brought against
QTC by any person or entity arising from your actions; your participation in the Activities; your use of
any of any of the facilities at any venue where scheduled Activities are happening; and/or your use of
any of the services offered by any independent contractor at any venue where Activities are
scheduled, whether or not you were negligent.

For the same consideration, you also acknowledge and agree that:
(a) QTC is not responsible for and you have no claim against QTC for the loss of, theft of or damage to
any of your personal property at any club (and, for greater certainty, this includes the loss of, theft of
or damage to any of your personal property at or in the parking lot at any Club), regardless whether
such loss, theft or damage is caused by an act of God, natural occurrence, QTC’s employees, other
Members, guests or any other person at the club;
(b) any entitlement to damages which you may have against QTC for any claims brought under this
Agreement shall not exceed amounts paid by you under this Agreement. I also agree to the QTC rules
as outlined by QTC, a copy of which is available upon request.

PARTICIPANT DECLARATION
I have read, understood and agree to be bound by the tournament entry conditions and freely give the
consents set out therein. I warrant that all information provided to TBT Inc and the GLTA and the
Event Host is true and correct. In participating in the tournament, I consent to being bound by the
listed policies.
