Life Members
Life Membership is the highest honour MUDTA can bestow upon an individual. It recognizes those who have demonstrated outstanding service and a long-term commitment to the values and growth of our association. Only financial members who have rendered distinguished service to the Association and who have been a financial member of the Association for a minimum of ten (10) years, are eligible to be elected as Life members of the Association.
Know someone who you think should be a life member?
Complete the nomination form together with supporting evidence documenting the contributions of the nominated person and email or handed to a committee member. Submission must be signed by two Ordinary Members. This form should reach the Secretary in time for announcement at the last meeting of the Management Committee prior to the Annual General Meeting.
Submission Instructions
Review Process: All nominations are reviewed by the Committee.
Confidentiality: Please ensure the nominee is not informed of the nomination to avoid disappointment should it not proceed to the final stage.
Deadline: Please return this form to the Secretary before the last committee meeting before the AGM.
